Can one orthodontic appliance treat all orthodontic issues?

Trying to treat all orthodontic situations with one appliance is like expecting a craftsman to build a house with one tool, or a mechanic to repair a vehicle with one wrench. Why are vacuum formed retainers so popular?

Vacuum formed retainers, also called “Tru Tain’s”, or “Essix” retainers have gained popularity over the standard wire and acrylic retainers. The vacuum formed retainers are clear, similar in appearance to an aligner tray, and patient acceptance is very high. That acceptance means patients are more likely to wear their retainers as instructed, thus helping their orthodontic correction hold. The wire and acrylic retainers generally are thicker and may require a longer period of adjustment, such as when speaking, when compared to a thinner vacuum formed retainer. The wire and acrylic retainers were designed to move teeth, specifically to close band spaces. Band space is the space between the teeth that may occur after orthodontic bands are removed. The wire and acrylic retainer may allow the orthodontist to close the spaces. Many orthodontists use a bonded orthodontic appliance, and place few or no orthodontic bands on the teeth. With bonded orthodontic braces, most spacing between the teeth will be closed prior to braces removal, so a vacuum formed retainer will suffice. The vacuum formed retainer is made from a model produced after the orthodontic appliances are removed, and is meant to hold the teeth in whatever their position at the time of braces removal. Arch wires, are they all the same?

.012, .016, .018, Hike! Is that a football play? No, it is a sequence of arch wire sizes that Dr. Larry Rose and Dr. Mohni Malik use to move teeth. Did you think braces move teeth? They don’t. The arch wires move teeth. Who thinks about arch wires? Arch wires get no respect! Dr. Larry Rose and Dr. Mohni Malik, the orthodontists at Houston Area Orthodontics, one of the best orthodontic practices in the Houston, TX area, give arch wires tons of respect! Are all arch wires the same? Not even close! The arch wires used at Houston Area Orthodontics are collectively referred to as “Nitinol” arch wires. To be more specific, the arch wires are heat activated “Nitinol” arch wires. Why heat activated? The force application using a heat activated arch wire is gentle and gradual. The arch wires apply the tooth movement pressure as they are warmed by a heat source, which is the mouth! Standard arch wires that are non-heat activated exert similar forces whether the arch wire is in the mouth or at room temperature. What that means to the patient is as soon as the non-heat activated arch wire is placed, the teeth are instantly loaded, and that can lead to some very sore teeth!
